For first-time visits or follow-up appointments
《For first-time general outpatient visits》 Please bring the following documents.- Identification document (driver’s license or passport)
- Medical insurance card
- Patient Registration Form and First Visit Form
- Please make an appointment by phone.
- For those visiting our clinic for the first time, please fill out the “Patient Registration Form (4 pages)” and the “First Visit Form (Internal Medicine).” You can download them at our download section and complete them in advance. If you are unable to download them in advance, we will provide you with the documents at the reception on the day of your visit.
- Please arrive 30 minutes prior to your scheduled appointment time on the day of your visit. We will collect the documents you brought at the reception.
- If you have a co-payment, please make the payment at the reception before the consultation.
- Please bring your mother and child health handbook (boshi techo) as well as records of all previous immunizations.
- For those visiting our clinic for the first time, please fill out the “Patient Registration Form (4 pages)” and the “First Visit Form (Pediatric).” You can download them at our download section and complete them in advance. If you are unable to download them in advance, we will provide you with the documents at the reception on the day of your visit.
- Prepare the records of immunizations and the documents above and come to the reception desk of our clinic.
◎Before the health check-up, we will review the immunization records to confirm the need for any required vaccines. - After submitting all the necessary documents, we will schedule your infant and child health check-up or school health examination.
※Please note that the appointment date will be approximately one week after the submission of the necessary documents. - Please arrive 30 minutes prior to your scheduled appointment time on the day of the visit. You may be required to fill out additional forms.
- For school health examinations, we will provide you with the Illinois state-designated form to submit to the school after your visit.
- If the cost of the health check-up is not covered by insurance, please make the payment at the reception desk after the consultation.
【For follow-up visits】 《For general outpatient/infant and child health check-up visits》
- Please make an appointment by phone.
- Please bring your medical insurance card with you for each visit.
- If you have changed your address, please inform the reception.
- If you have Co-Payment, please make the payment at the reception before the consultation (applicable for general visits only).
- If the cost of the consultation/health check-up is not covered by insurance, please make the payment at the reception after the consultation.
- Please make an appointment by phone.
- Please bring your medical insurance card with you for each visit.
- If you have changed your address, please inform the reception.
- If the Illinois state-designated form is required, it will be created after the vaccination.
- If the cost of the vaccination is not covered by insurance, please make the payment at the reception after the vaccination.
※Please note: If you are visiting for a vaccination outpatient visit, it does not include a consultation with a doctor.
